Does it belong in the psychotherapy note or the medical record note?
I got a request for medical records what do I do?
This workshop is intended to support beginner to intermediate clinicians in their developmental journey related to documentation and it's relationship to HIPAA, the law and the emotional safety of the therapeutic container.
Objectives for this workshop:
•Learners will be able to name and locate two Federal Laws and one Federal web resource related to documentation maintenance and record keeping
•Learners will be able to identify the 18 components of the medical record
•Learners will be able to define the difference between a medical record session note and a psychotherapy note
•Learners will be able to describe how to respond to a request for medical records

Nick (he/him), is a professional counselor, accredited supervisor, and AASECT Certified Sex Therapist. Nick serves as the Chairperson for the AASECT Ethics Committee, is the founding member of the AASECT Gender Diverse Special Interest Group, and the recipient of the 2020 AASECT DEI Award.
Nick blends his unique lived and clinical experience to provide in-depth information and passionate humanity to the work.
His professional experience has been across the continuum of care. His experience in community behavioral health service provision, program development and leadership, Adminstrative Oversight of statewide community behavioral health services, forensics, substance use, and instructional development and training, as well as in private practice leaves him with a vast knowledge base. Combined with his unique perspective on the world as an AuDHD trans person, his experience and insights are bound to leave you with a new perspective and curiosities about your own approach to the work.
